Crisis intervention:
Available 24 hours a day. Sr Care management professional is able to step in and
take care or offer direction in the midst of a crisis. We advocate for our
clients in the emergency room offering peace of mind to family members in
knowing that the emergency situation will be handled. Hurricane/Disaster
preparedness plans available.
In- home comprehensive assessment and
evaluation:
Conducted by a licensed nurse/social worker an in-home visit includes screening
of the older adults’ functional status, cognitive status, nutritional status, in
home safety, supports and social systems, medication review, review of
comprehensive medical records and review of current medical conditions. Problems
and needs are uncovered. An individualized focused care plan is designed to
address these needs. Examples of resources can include: Transportation
arrangements, adult day care, in- home assistance, Medical equipment, escort to
necessary appointments, housekeeping and personal response systems.
Consultations:
By appointment counseling education and support can be provided regarding aging
issues and concerns.
Ongoing support and supervision:
Ongoing monitoring and supervision depending on need, daily, weekly, bi-weekly
monthly. Routine reports can be given to family members either on –line or
through verbal communication via phone regarding status and changes.
Home Modifications:
Contracting and supervision with home repair services. House modifications to
adapt equipment.
Provision and supervision of in-home
caregivers (companions and home health aides):
Selection of appropriate in-home care givers. Monitoring for compatibility, and
provision of appropriate service.
Placement in retirement communities,
rehabilitation facilities, assisted living facilities or nursing home,
advocacy in long term care facilities:
When appropriate our Geriatric Care Managers can select and secure placement in
the most compatible facility for the client’s needs. Support is given with
adjustment issues as a result of transitions from home to facility.
Relocation:
Complete relocation service logistics. Personally handled intimate details of
downsizing to a smaller residence or facility location.
Medication set ups and monitoring:
If necessary after skilled nursing benefit is exhausted, our nurses can provide
accurate medication set ups, medications, and monitor the administration of the
medications.
Bill paying and statement review services
SCMS provides an efficient accurate assistance with bill paying, review of
medical invoices, tax preparation and financial planners.
Maximization of entitlements:
Often the elderly client is unaware of the benefits available to them through
their medicare insurance plans and long term care policies. SCMS staff is able
to secure all benefits available to them for these services. Knowledge of
criteria for personal benefits.
Social interaction and support:
Provide social interaction in-home or at facility and assure participation in
therapeutic activities.
Referrals to other professionals:
Our care managers are well networked with the community support services
examples include: physicians, psychiatrists, eldercare attorneys, realtors,
financial planners, trust officers, etc.
Assistance with legal issues:
Support for power of attorney, health care surrogate, living will and estate
planning.
