Case Management Services

As professional geriatric care managers, we work with you to assess and develop the best possible care plan to meet the specific needs of the elderly or disabled adult. We are there to assure that the care plan is put into place.

Senior Care Management Services continues to monitor the elderly or disabled individual's health and well-being and provides medication management when needed.

We will assist with routine services, including bill paying, transportation needs, coordination of medical care, and medical claims filing.

We will accompany the client to physician visits and diagnostic tests.

Families are kept apprised of their loved ones’ on-going progress through emails and/or monthly telephone contact. Of course family members my call us with questions and concerns.

We regularly reassess a client's situation to ensure his or her needs are understood and met.

As Professinal Geriatric Care Managers we:

• Conduct elder care-planning assessments to identify problems, eligibility for assistance, and need for senior services;
• Screen, arrange and monitor in-home help or other home health care services;
• Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets;
• Provide crisis intervention;
• Act as a liaison to families at a distance, making sure things are going well and alerting families to problems;
• Assist with moving an elderly person to or from assisted living facility, a retirement complex, care home, or nursing home;
• Provide consumer education and advocacy;
• Offer counseling and support.
